Resident Camp Forms
CampDoc

CampDoc is an online system where parents/guardians can submit information about their camper's health history, buddy request and more. You'll receive an email from CampDoc with information on how to access your online account to complete this information. 

CampDoc records must be completed online within two weeks after registering for camp.

CampDoc Welcome Letter

Health History Profile | This must be completed in your CampDoc account.

Immunization Record | This form must be completed, signed and uploaded to CampDoc.

Immunization Exemption Request | To be completed for all campers who are not fully immunized based on age requirements. This form must be completed, signed and uploaded to CampDoc.

Camper Agreement Form/Behavior Policy | This information must be reviewed, and signed off by the camper and parent/guardian in your CampDoc account.

Notice of Privacy Practices Information | After reading this document, check "yes" in the Privacy Authorization section in CampDoc, found under Step 10 of the CampDoc online form.

Parent Guide to Resident Camp | Includes information about resident camp, suggested packing list, camp mail, and more. Please review with your camper.

Purchase Bus Transportation | This form allows camper to have transportation. This form must be received 2 weeks prior to your camp date.

CampDoc Medication Form | Print this form from your CampDoc account and bring it along with needed medications. 

Financial Assistance

Partial financial assistance may be requested for any registered Girl Scout living in 32 counties served by Girl Scouts of Northeast Texas. GSNETX makes every effort to assist as many girls as possible from our allocated funds. Families with financial situations that may prevent them from participating in the resident camp program can request assistance for a portion of session and transportation fees, minus the required $100 deposit. If you're requesting Financial Assistance for Resident Camp:

1. Choose your registration preference:

Before May 1 Register online and DECLINE the bus option. If you need transportation to camp, select the bus option during step 2. Choosing bus transportation online will charge you the non-refundable $90 bus fee.
After May 1 Complete the Resident Camp Registration Form and submit your $100 deposit. If you need transportation to camp, select the bus option during step 2.

2. Complete the GSNETX Financial Assistance Online Request. If you need transportation to camp, be sure to answer YES to request the bus. Incomplete requests will not be processed. 

3. If financial assistance is granted, you'll get an email with the amount of financial assistance you'll receive. Then login to your eBiz account to confirm the remaining balance owed and pay in full online. The remaining balance is due by May 1, 2018.

Need to cancel your registration? Email customercare@gsnetx.org as soon as possible. The camp fee (minus a $100 cancellation fee) will be refunded if cancellation is received at least 30 days before the camp session start date.
